    Rule 1455 .  (1)  "Atm ospheric press ur e" mean s th e press ur e of air at  sea level, usua lly

    14.7 p.s.i.a . (1 atm osphere) or zero p.s.i.g .

    (2)   "Auger tunn el" mean s that A tunn el w h ich THAT is excavat ed by use of a cont inu ous flight au ger system, with or with out a sleeve or oth er type of liner.

    (7)           DECANTING MEANS A METHOD USE D FOR DECOMPRESSING UN DER EMERGEN CY CIRC UMST ANCES. IN THIS PROCEDURE, THE EMPLOYEE S ARE BROUGHT TO ATMOS PHE RIC PRESSUR E WITH  A VERY HIGH GAS TEN SION IN THE TISS UES AND THEN IMMEDIATELY RECOMPRESSED IN A SECOND  AND SEP ARATE CHAMBER OR LOCK .

    (8)   EMERGEN CY LOCK MEANS A LOCK DESIGN ED TO HOLD AND PE RMIT THE QUI CK PASSAGE OF AN EN TIR E SHIFT OF EMPLOYEE S.

    (9)   HIGH AIR MEANS AIR PRESSUR E USE D TO SU PP LY POWER TO PNEU MATIC TOOLS AND DEVICES.

    Rule 1456 .  (1)  "Man lock" mean s a cham ber thr ough which employees pass from 1 air press ur e environm ent int o an oth er.

    (2)     MATERIALS LOCK MEANS A CHAMBER THROUGH WHICH MATERIALS AND EQUIP MEN T PASS FROM ONE AIR PRESSUR E EN VIRONM EN T INTO ANOTHE R.

    (3)   (2) "Occu pied tunn el" mean s an y tunn el ent ered by 1 or more employees.

     

     

    (4)   (3) "Pr ess ur e" mean s a for ce acting on a un it ar ea, usua lly shown as poun ds per squar e inch (p.s.i.).

    (5)     (4) "Qua lified employee" mean s one  who,  by  extensive  kn owledge ,  tra ining,  an d expe rience, ha s su ccess fully demonstrat ed th e ability to solve or resolve problems relat ing to th e subject matt er, th e work , or th e pr oject .

    (6)   (5) "Safety screen" mean s an airt ight an d wat ert ight diaphra gm placed vert ically across th e uppe r part of a com pressed air tunn el between th e face  an d  bulkh ead  in  order  to restra in sudde n floodin g of th e crown of th e tunn el between th e  safety  screen  an d  th e bulkh ead, thu s providing a mean s of refuge an d exit from a floodin g or flooded tunn el.

    (7)   (6) "Sha ft " mean s a vert ical or inclined opening excavat ed below groun d level.

    (8)   (7) "Tunn el" mean s a genera lly horizonta l excavat ion or facility more than l4 inches in diam eter, width , or height an d more than 6 feet in length below groun d or wat er. Fa cilities used for th e acquisition of minera ls, ores, an d foss il fuels, which ar e comm only kn own as mines, ar e excluded from th is definition.

    Rule 1461 . (1) Befor e th e start of a tunn el, as defined IN R 408 .41456 , WHICH IS 24 inches or more in diam eter, height or width , AND which will be occu pied by an employee, a report prepar ed by th e employer per form ing th e tunn el excavat ion sha ll be sent to th e Michigan Dep artm ent of Consum er an d Industr y Services, Constru ct ion Safety Divisi on, 7150 Harr is Drive, P.O. Box  30015  30645 ,  Lan sing,  Michigan  48909 -8145 ,  an d  th e Mich igan Dep artm e nt of Pu blic H e a lth , O ccu p at iona l H e a lth Divisi on , P .O . B ox 30035 , 3500 N .     R 408 .41462   Sa fe ty gene rally.

    Rule 1462 . (1) ONCOMI NG SHIFTS SHALL BE IN FORMED OF ANY HAZARDOUS OCCURREN CES OR CONDITIO NS THAT HAVE AFFE CTED OR MIG HT AFFE CT EMPLOYEE SAFE TY, INCLUDING LIB ERATIO N OF GAS, EQUIP MEN T FAILUR ES, EARTH OR ROCK SLID ES, CAVE-INS , FLOODI NGS, FIRES, OR EXPLOSIO NS.

    (2)   (1) A safe mean s of egress an d a ccess to all work ar eas sha ll be provided an d ma inta ined free of ha zar ds.

    (3)   (2) Form scra p mat erial, lum ber that ha s protru ding na ils, an d all oth er debris sha ll be kept clear ed from th e work ar eas, passageways, sta irs, locks , an d chan ge houses.

    (4)   (3) Combustible debris sha ll be removed daily dur ing th e cour se of con stru ct ion.

    (5)     (4) When a hau lage roadbed con sists of tra ck an d wood ties, a walkway sha ll be provided. The walkway sha ll be a minimum of 2 2-inch plank s, side by side, abutt ed, joined, an d secur ed to a tie or oth er equivalent mean s. If space is not adequat e for 2 2-inch by 10- inch plank s, THEN th e walkway sha ll be as wide as space perm it s.

    (6)    (5) When work is not being per form ed, a ccess to an  un dergroun d  opening sha ll be cover ed, bulkh eaded, fenced off, or restr icted by gat es or door s an d appropriat ely posted.

    (7)   (6) Any section of tunn el that is not in use sha ll be barr icaded to prevent ingress by an unauth orized employee.

    (9)   (8) The power sour ce to a tunn eling ma chine sha ll be disconn ected or lock ed out when an employee is work ing in th e ar ea of th e cutt ing head or per form ing ma int enan ce work on th e tunn eling ma chine where motion could cau se an injur y.

    (10)   (9) An ar ea subject to subside nce that is ha zar dous to an employee sha ll be fenced an d appropriat ely posted.

    (11)     (10 ) Ea ch operat ion sha ll ha ve a check-in an d check -out system that will  provide positive ident ificat ion of an employee by num ber or nam e an d will ident ify th e locat ion of each employee who is un dergroun d. An a ccurat e recor d sha ll be kept on th e sur face. However, a check-in  an d  check-out  system  is  not  required  when  th e  con stru ct ion  of un dergroun d facilities that  ar e des igned for human  occu pan cy ha s been com pleted so that

     

     

    th e perman ent environm enta l contr ols ar e effect ive an d th e rema ining con stru ct ion activity will not cau se an y environm enta l ha zar d or stru ctura l fa ilur e with in th e facilities.

    (a)     Air monitoring.

    (b)     Vent ilat ion.

    (c)     Illum inat ion.

    (d)    Commun icat ions.

    (e)     Flood contr ol.

    (f)      Mechan ical equipment .

    (g)     Persona l protective equipment . (h) Explosives.

    (i)     Fire prevent ion an d protection.

    (j)     Em ergency pr oced ur es, including evacuat ion plan s an d check-in an d check -out systems.

    (15)   (14 ) Ea ch employee sha ll be issued a copy of th e pr oject 's genera l safety ru les befor e an employee comm ences work at th e pr oject .

    (16)     (15 ) Ea ch employer sha ll des ignat e a qua lified employee who is resp onsible for administering th e  safety  program .  A writt en  recor d  sha ll  be  ma inta ined  of th e  safety tra ining program .

    (17)    (16 ) Before an employee ent ers a tunn el where th e atm osphere might be ha zar dous due to a con dition such  as  a  deficiency  of  oxygen,  or  might  be  toxic  in  excess  of  th e ma ximum a llowable limits, th e tunn el sha ll be tested an d th e results sha ll be recor ded AS PRESCRIB ED IN R 325 .62991 ET SEQ., OCCUP ATIO NAL HE ALTH STANDARD PART 665 UN DERGRO UN D CONSTRUCTIO N, CAISSONS , COFFE RDAMS, AND COMPRESSED AIR. The records sha ll be ma inta ined at th e jobsite. If th e atm osphere is ha zar dous, eith er su fficient vent ilat ion to eliminat e th e  ha zar d  sha ll  be  provided  or respi rat ory equipment as prescribed by th e dep artm ent of con sum er an d industr y services sha ll be worn .

    (18)   (17 ) If an atm osphere is foun d to be explosive, THEN spark s, flam e, an d oth er sour ces of ignition sha ll be prohibited an d vent ilat ion sha ll be provided unt il th e ha zar d ha s been reduced an d ma inta ined at or below th e ma ximum  allowable limits as prescribed by th e dep artm ent of con sum er an d industr y services.

    (19)       AN OCCUPIE D AUGER OR PIPE J ACKI NG TUNNE L SHALL BE MONI TORED FOR AIR QUALIT IMMEDIATELY  BEF ORE EN TERING THE TUNNE L AND DURING THE PE RIOD OF OCCUP ANCY IN THE TUNNE L.

     

     

    R 408 .41463   Eme rgency  provisions ; plans ; equip men t; rescue  cre w s .

    Rule 1463 .  (1)  A plan to evacuat e a tunn el in an emergency an d th e procedur es to carr y out th e plan sha ll be developed an d ma de kn own to th e employees an d to th e rescue team .

    (2)     An employer sha ll ensur e that  rescue team s ar e fam iliar with con ditions at th e jobsite.

    (3)      On jobsites where 25 or more employees work un dergroun d at one t ime, an employer sha ll provide, or mak e arran gement s in advan ce with loca lly available rescue services to provide, not less than 2 5-person rescue team s. One of th e team s sha ll be on th e jobsite or with in 1/2 hour of tra vel t ime from th e entr y point an d th e oth er team sha ll be with in 2 hour s of tra vel t ime from th e entr y point .

    (4)      On jobsites where less than 25 employees work un dergroun d at one t ime, an employer sha ll provide, or mak e arran gement s in advan ce with loca lly available rescue services to provide, at least 1 5-person rescue team . The team sha ll eith er  be on  th e  jobsite or  be with in 1/2 hour of tra vel t ime from th e entr y point .

    (5)        On jobsites where  flamma ble  or  noxious  gases  ar e  en count ered  or  ant icipat ed  in ha zar dous quant it ies, rescue team  members  sha ll  pra ct ice  donn ing  an d  using  self- conta ined breath ing apparatu s month ly.

    (6)     An emergency hoisting facility, such as a mu d box or a cran e, sha ll be readily available at a sha ft that is used as a mean s of egress , un less a hoisting mean s is provided that is indepe ndent of an electr ical power fa ilur e.  THE  HOISTI NG  MEANS  SHALL  BE DESIGN ED SO THAT THE LOAD HOIST DRUM IS POWERED IN BOTH DIR ECTIO NS OF ROTATIO AND SO  THAT THE  BRAKE  IS  AUTOMATIC ALLY APP LIED UP ON P OWE R RE LE ASE OR F AILUR E .

    (8)   (9) Self-conta ined breath ing apparatu s sha ll be ma inta ined in good operat ing condition. Em ployees sha ll be tra ined in it s use.

    (9)    (10 ) There sha ll be a rescue crew for each shift of all un dergroun d operat ions. The rescue crew sha ll be tra ined in rescue pr oced ur es, th e use an d limitat ions of a breath ing apparatu s, an d th e use of fire fight ing equipment . The crews sha ll be retra ined at least once each year . Loca l fire an d police personn el ma y be used as rescue team s for tunn el operat ions. If loca l personn el ar e to be used , THEN th e employer sha ll arran ge for such assistan ce befor e th e start  of th e pr oject .

     

     

    (10)   (11 ) The followin g minimum rescue equipment sha ll be provided at th e top of th e sha ft : (a) Four un it s of 1/2-hour -rat ed, air-supplied respi rat ors.

    (b)     Four additiona l un it s of 1/2-hour -rat ed air bott les.

    (c)       Four bur eau of mines flashlight s or lant ern s with additiona l batt eries for each light . The flashlight s sha ll be as prescribed in part 20 of subcha pter B of THE PROVISIONS OF THE CODE OF FE DERAL REGULATIO NS , TITL E 30 , MINE RAL RESOURC ES, REVISED J ULY 1, 2000 : PART 1-199 , 30 C .F .R . p art s  0 -199 ,  m in e ra l  r es our ces , 1988 edi t ion , which is adopted by reference in subru le (7) (8) of th is ru le.

    (d)    Two 2A-10BC fire extinguishers.

    (e)     One str etcher, wire baske t type with slings atta ched.

    (f)      One fire blank et.

    (g)     One 10-ton han d hydrau lic rescue kit.

    (11)   (12 ) At least 1 employee sha ll be on dut y above groun d when an employee is work ing un dergroun d. The primar y dut y of th e employee who is above groun d sha ll be to secur e imm ediat e aid for an employee who is un dergroun d in case of an emergency.

     

    R 408 .41464   Communication    system ; location ; signa ls .

    Rule 1464 .  (1)   In tunn el that  is more than  225 feet long, a commun icat ion system sha ll be provided at all of th e followin g locat ions:

    (a)     The work ing face.

    (b)     The top of th e sha ft .

    (c)    The bott om of th e sha ft .

    (d)    The hoisting stat ion, if provided .

    (e)     Ea ch 1,000 feet of tunn el.

    (f)       The  office, if provided .   A public telep hone  or  oth er  commun icat ion  system  sha ll be provided or available to each tunn el pr oject to secur e out side emergency help.

    (7)        Fire-resis tant hydrau lic fluids sha ll be used  in  hydrau lically actuat ed  un dergroun d ma chinery an d equipment .     (8)      An approved 2 A-10BC 4A:40B:C RATING fir e extinguisher or equivalent  protection sha ll be provided at th e drive pulley of an un dergroun d conveyor an d at 300 -foot int ervals along th e belt. A minimum of 2 2A-10BC approved fire extinguishers sha ll be provided at th e tunn eling ma chine.

    (9)         A press ur ized tunn el in which com bustible mat erials ar e stored or used sha ll be equipped with a 2-inch minimum diam eter wat er line with an out let that is conn ected to a 1 1/2-inch nomina l diam eter fire hose which is capable of reaching th e com bustible mat erials.

     

     

    The wat er supply sha ll be of su fficient volum e an d press ur e to efficient ly operat e th e type of nozzle used on th e fire hose for a minimum of 1 minut e. Fire extinguishers  ma y  be substitut ed for th e wat er an d fire hose if th ey meet th e requirement s for th e wat er service.

    (2)      The roadbed , ra ils, joint s, switches, frogs, an d oth er element s of th e tra ck of a hau lage road sha ll be con stru cted, insta lled, an d ma inta ined in a mann er that is con sistent with th e speed an d type of th e hau lage operat ions to be con ducted.

    (3)      A tra ck switch sha ll be provided with a lock ing or spring-loaded thr own bridle bar an d guar dra il. The switch thr ow, where possible, sha ll be placed on th e clearan ce side, an d th e switch thr ow sha ll operat e para llel to th e hau lage road.

    (7)         W h e n IF an employee is ra ised or lowered in  a  sha ft ,  THEN  all  oth er  hoisting operat ions in th e sha ft sha ll be stopped unt il th e employee ha s disembark ed at th e bott om or top of th e sha ft .

Ea ch leg of th e suspe nsion system sha ll be indepe ndent wire rope that ha s han d-tu cked eye splices or swed ged fitt ings on each end. Wire rope clips sha ll not be used. The indepe ndent suspe nsion system sha ll be atta ched to th e work plat form using proper size screw pin sha ckles.

    (f)     Ha ve each leg of th e indepe ndent 4-point suspe nsion system at a 30-degree an gle from th e vert ical.

If a screw pin sha ckle is used in place of a gat ed h ook , THEN th e 5/8-inch wire rope safety line is not required.

    (j)     Be easily ident ifiable by high-visibili ty color or mark ing.

    (3)        Befor e a work plat form is  used  on  a  jobsite,  it  sha ll  be  load-tested  to 2  t imes  th e ma ximum int ended load.

    (7)         Fr ee-sp oolin g is prohibited  when  using  a  work  plat form  to  lower  personn el.  The ma ximum rat e of tra vel sha ll be 100 feet per minut e.

    (8)      All sides of personn el cages sha ll be enclosed by 1/2-inch (12.70 mm ) wire mesh, which sha ll not be less than no. 14 gau ge or it s equivalent , to a height of not less than 6 feet (1.83 m). When th e cage or skip is being used as a work plat form , it s sides ma y be reduced in height to 42 inches (1.07 m) when th e con veyan ce is not in  motion.  All personn el cages sha ll be provided with a positive-lock ing door that ONLY OPEN S IN WARD. d oes n ot ope n out war d .

     

     

    (9)     The ra ils on 1 side of personn el cages sha ll be removed an d a cha in sha ll be insta lled in place of th e top ra il to provide a door opening. The cha in sha ll be secur ely fastened dur ing all tra vel an d only be opened dur ing a ccess to, or egress from, th e work plat form .

    (10)     Only han d an d porta ble powered t ools sha ll be perm itt ed on th e work plat form .

    (11)     Flamma ble or com bustible liquids or gases sha ll not be perm itt ed on th e work plat form if th e plat form is occu pied by an employee or employees.

    (12)       Plat form s sha ll not be used dur ing adverse weath er con ditions that cou ld a ffect th e safety of employees.

    (13)     There sha ll be a commun icat ion system, which ma y be a han d signa l, a telep hone wire, or a selective frequency ra dio system, between employees on th e work plat form an d th e operat or of th e cran e. The system sha ll be in com plian ce with both of th e followin g provisions:

    (a)          If han d signa ls ar e being employed an d employees ar e being ra ised , lowered, or positioned an d ar e not in cont inu ous sight of th e operat or of th e cran e at all t imes, THEN th e employer sha ll des ignat e an employee, who sha ll not be on th e work plat form , to be th e signa lperson.

    (b)     The signa lperson sha ll not be assigned an y oth er dut ies while th e work plat form is in a suspe nded position with employees on it an d sha ll rema in in a  position  so that  both  th e work plat form an d th e operat or can be seen at all t imes.

    (14)      The cran e sha ll be inspected daily befor e being used with a work plat form . All of th e followin g com ponent s sha ll be inspected:

    (a) The wire rope.  (b) The h ook.  (c) The brak es.  (d)  The boom .

    (e) Any oth er mechan ical an d rigging equipment that  is vita l to th e safety of th e operat ion.

    (15)       Any stru ctura l or mechan ical defect that cou ld adversely a ffect th e safe operat ion of th e cran e sha ll be corr ected befor e an operat ion that  ut ilize s a work plat form begins.

    (16)     Inspe ct ions sha ll be logged an d th e recor ds ma inta ined on th e jobsite.

    (17)         The operat or of a cran e that  is  used  to  ra ise  or  lower  a  work  plat form  sha ll  be auth orized by th e employer an d properly qua lified to per form th e operat ion.

    (18)      A properly qua lified cran e operat or sha ll not be auth orized to ra ise or lower a plat form un less th e operat or ha s ha d not less than  8 hour of expe rience in  th e operat ion  of th e SPE CIFIC cran e OR ON A CRANE OF THE SAME TYPE AND DESIGN .

    (19)      A cran e that is used to ra ise or lower a work plat form sha ll not be with in 25 feet of an overh ead energized power line at th e closes t point of conta ct .

    (20)     When a cran e is being used with a work plat form , an oth er load sha ll not be atta ched to th e work plat form .

    (21)       Only a cran e that is equipped with a boom that ha s a power contr ol lowering system sha ll be allowed to ra ise or lower a work plat form .  The cran e boom sha ll not be live.

    (22)      The operat or of th e cran e sha ll rema in at th e contr ols with th e engine runn ing when an occu pied work plat form is in a suspe nded position.

    (23)     The load line of a cran e that is used to ra ise or lower a work plat form sha ll be equipped with a swivel to prevent an y rotat ion of th e work plat form . The use of nonspin wire rope is prohibited.

    (24)      Neith er th e load nor th e boom sha ll be lowered below th e point where less than 4 full wra ps of rope rema in on th eir respe ctive drum s.

     

     

    (25)       A cra wler cran e that is used to ra ise or lower a work plat form sha ll be set on a firm base an d ch ock ed to prevent movement .

    Rule 1479 . If th ere is a dan ger of ra pid floodin g in a tunn el that ha s a bore of 16 feet or more in diam eter, th en both of th e followin g sha ll be provided:

    R 408 .41483   Caissons  used  unde r pressu re.

    Rule 1483 . (1) A caisson used un der press ur e sha ll be subjected to a hydrostat ic test equa l to 200 % of th e work ing press ur e befor e being used on a job, at which press ur e it sha ll be tight . The caisson sha ll be stam ped  on  th e out side  shell  about  12  inches  from  each flan ge to show th e press ur e to which it ha s been subjected.
