In the event of loss, destruction, or mutilation of a certificate or permit, the person to whom the certificate or permit was issued may obtain a replacement upon furnishing satisfactory proof of loss, destruction, or mutilation. Application for a replacement certificate or permit shall disclose the following information:

(a) ) Name and address of applicant.

(b) An explanation of the loss, destruction, or mutilation of the original certificate or trainee permit.
