  • (1) An applicant or licensee shall give written notice to the department within 5 business days of any change in information submitted as part of an application for initial or renewed licensure.

    (2)   A license is not transferable.

    (3)   The current license shall be posted in a conspicuous public place in the hospice and its hospice residence, if applicable, office. For purposes of this rule, the term "license" includes a limited license or a temporary permit.
