  • (1) Take-home medication shall be formulated in such a way as to minimize parenteral abuse and shall be packaged pursuant to section 3 of the poison prevention packaging act, 15 U.S.C. S1472.

    (2)  Take-home  medication  shall  be   labeled      with    all       of       the       following information:

    (a)   The name of the medication.

    (b)   The treatment center's name, address, and phone number.

    (c)   Client name or code number.

    (d)  Medical director's name.

    (e)   Directions for use.

    (f)  Date to be used.

    (g)   A cautionary statement that the drug should be kept out of the reach of children.
