(1) A program shall establish a control register which includes all of the following information:
(a) The name of the client.
(b) The date and time of client's arrival.
(c) The client's means of arrival and by whom transported.
(2) A program shall keep client records which shall include all of the following information:
(a) The medical exam and evaluation.
(b) The treatment plan.
(c) An evaluation of social and psychological needs.
(d) The discharge summary.
