An approved service program shall have a written description of its triage process. There shall be documentation that this process is developed in conjunction with a licensed physician. The description shall include all of the following:

(a)   The method used in determining the level of urgency of need of each individual client.

(b)     Identification   of the services   to   be    performed,   including transportation if necessary.

(c)   The method of assigning the priority of required services.
