The written patient care policies shall be made available for review on request to all of the following:

(a)   A patient.

(b)   Physician.

(c)   Nursing personnel.

(d) Next of kin.

(e)   Guardian or designated representative.

(f) Agency or organization responsible for placing or maintaining the patient in the home.
