  • Any person who believes that a provision of the code relating to nursing homes, any other law administered by the department relating to nursing homes, or these rules has been violated may file a complaint with the department. The complaint shall include a statement  of   the  facts, without repetition, upon which the complainant relies to fully note his or her complaint and may include a statement of the relief requested.
