When a patient has a referring health care provider or a patient has named a health care provider, the stereotactic breast biopsy facility shall provide a written report of the stereotactic breast biopsy procedure, including the items listed in R 325.5679, to that health care

provider not later than 30 days after the date that the stereotactic breast biopsy procedure was performed.
