  • A stereotactic breast biopsy image shall have the following information indicated on it in a permanent, legible, and unambiguous manner and placed so as not to obscure anatomic structures:

    (a)   ) Name of patient and an additional unique patient identifier.

    (b)   Date of the procedure.

    (c)   ) Designation of left or right breast.

    (d)  Cassette identification, if applicable.

    (e)   ) Stereotactic breast biopsy unit identification if there is more than 1 unit in the facility.
