(1) A person who desires to submit a request for declaratory ruling under 1969 PA 306, MCL 24.201 et seq. shall make that request in writing. The request shall be filed with the Michigan Board of Nursing, Department of Consumer and Industry Services, 611 West Ottawa, P.O. Box 30670, Lansing, MI 48909.

(2) The board or its authorized representative may require the person to submit additional information necessary to make an appropriate resolution of the matter.
