An applicant for a respiratory therapist license, in addition to meeting the code and the administrative rules promulgated under the code, shall comply with both of the following provisions:

(a) Before December 1, 2006, submit a completed application on a form provided by the department together with the requisite fee according to provisions of section 18709(2).

(b) Submit documentation of a respiratory therapist certification or registration credential from the nbrc.
