l. (1) An applicant whose license has lapsed may be relicensed upon submission of the appropriate application, fee, and documentation of completing 45 hours of approved continuing education within 3 years immediately preceding the application for licensure. If the applicant is registered or licensed in any other state or states, the applicant shall submit documentation that the registration or license is unrestricted and in good standing at the time of application
