An agency shall   have and follow a   current written program statement that includes all of the following information:

(a)   Types of foster care provided.

(b)   Age, race, ethnic background, and specific characteristics of children served.

(c)   Types and numbers of foster homes needed.

(d) Types of services provided to foster families.

(e)   Geographical area covered.
