An agreement to redeem the liability of the carrier, second injury fund, self- insurers’ security fund, or silicosis and dust disease fund shall be submitted on form 556, agreement to redeem liability. The agreement shall be accompanied by a report, approved by the employee, from a licensed physician stating, in detail, the findings of a recent examination.
