d. Application for group coverage, as contemplated in section 611 of the act for the express purpose of establishing a group self-insurers' fund, to be administered under the direction of an elected board of trustees and to provide workers' compensation coverage for a group of private employers in the same industry or for public employers of the same type of unit, shall be made to the bureau. The application shall be made on a form prescribed by the bureau and shall contain answers to all questions. Answers shall be given under oath.
