A stereotactic breast biopsy facility shall prepare a written report of the results of each stereotactic breast biopsy procedure. The stereotactic breast biopsy report shall include all of the following information:

(a)   The name of the patient and an additional unique patient identifier.

(b)   The date of the procedure.

(c)   The name of the stereotactic breast biopsy physician who conducted the procedure.

(d) The procedure performed.

(e)   ) Designation of the left or right breast.

(f) ) Description and location of the lesion.
